Case Management
Our food security programmes, empower the community to embrace health and well-being through better lifestyle choices. The monthly food distribution programme (i.e. PantryPlenish) provides dry food rations to low-income families and aged seniors in Chinatown, Jalan Kukoh, and York Hill.
Fortnightly distributions of breakfast items (i.e. BreadWalk) provide relief to under-resourced families and seniors before they receive their next food ration pack. These distributions also grant us the opportunity to main regular contact and monitor their well-being.
Groceries such as fresh chicken and fish, along with a variety of fruits and vegetables (i.e. FreshFood) are distributed to enable families and individuals an opportunity to eat fresh produce.
We provide personalised support to individuals and families facing challenges by assessing their needs, coordinating services, offering guidance, and working towards stability, independence, and overall well-being.
After the assessment, we also offer relevant interventions and resources, journeying alongside them to help improve their lives.
Our emergency relief fund provides short-term financial support to low-income breadwinners who have lost their jobs, experienced income disruptions, or faced personal income lost.

All referrals are carefully assessed on a case-by-case basis.
